When you Click "Register/Sign in yourself" Button Display Mobile Number Register Page 

Enter Your 10 Digit Mobile Number in this Box and Click the "Get OTP" Button. 

With in 10 Second you Recieve OTP Code SMS From NHPSMS To your Register Mobile Number.

Enter the OTP Code and Click the "Verify & Proceed" Button.

Step - 3

When you Clicked "Verify & Proceed" Button After Display this Page Page Click the "Register Member" Button.

Click " Register Member " and Display Register for Vaccination Online Form.Per Mobile Number Allowed to Book 4 Members Only.

Choose Your Photo ID Proof 

If you Choose Aadhaar Card for Photo ID Proof 

1.You Fill Your Aadhaar Card Number 

2.Enter Your Name 

3.Select Your Gender

4.Enter Your Birth Year 

Finally Check you fill this Form Correctly and Click the "Register" Button 

Step - 4

After Register Show Your Registration Detail and Schedule Option.you Click the Schedule Button Choose your Nearest Center Choose the Date & Time.

After Click the Schedule Button Display the Appointment Booking Page Click The "Search the District" Button .

Select Your State and District after Click the Search Button 

Step - 5

After Click the Search Button its Show  your Age Category,Cost Option,Vaccine Option,Your Nearest Vaccine Centers and Vaccine Available Detail.

For a Example you Need to fix 12 june 2021 Date you click the Green Box (Vaccine Available Box) 

You Choose the Time Slot and Click the Confirm Button 

After Click the Confirm Button Display the Your Vaccine Appointment Confirmation Details and Right Side Top you see the Download Button You Click the Download Button Your Registration PDF File Download on your Device Sucessfully.
